Lord Strain is a portrayal of the Fear of Disease from the blog Horsemen. He is actually the third Horseman of the Apocalypse, outcast for unknown reasons. He became a Terrorspawn, with some help from Akuma, and seeks vengeance against the other Horseman and their children.

Appearing as a cloaked man, Lord Strain has black bandages covering his face to conceal a gruesome injury. He wields a gun and a long sword, both which cause illness when he attacks. In his true form, he grows an ivory beaked mask and bat-like wings. Additionally, his gun transforms into a gauntlet which can be fired at people.
